1. NAME OF PROGRAM: Case Management Services for Adults
2. PURPOSE OF PROGRAM: Case management is the planning and directing of services to assure that an individual's needs are met directed towards the goal of maintaining health and safety. This includes:
- Assessing the individual's current needs.
- Developing a plan to meet the needs.
- Taking actions to assure that services are implemented as identified.
- Evaluating the effectiveness of the plan and services provided.
3. APPLICATION PROCEDURES: Application for assistance with case management services is made with the Cabarrus County Department of Social Services through the Adult Service Intake unit. Applications may be made by phone or in person.
4. ELIGIBILITY REQUIREMENTS: The eligibility requirements include:
- The individual must be in need of assistance in managing day to day activities and securing services from community resources.
- This service is provided without regard to income and eligibility is based solely on the documented need for the service.
5. OTHER PROGRAM DETAILS: Activities include locating and coordinating sources of help from within the family and community. Components of this service incorporate assisting with money management - the agency acts as representative payee for benefits from the Social Security Administration, arranging for health care and coordinating transportation services to obtain health care, and arranging for in-home aide services to assist with personal care and home management tasks.
