SERVICES

ADULT DAY SERVICES

The Cabarrus County Senior Centers provide Adult Day Care and Adult Day Health services to Cabarrus County residents who are 60 years old or older. These services are provided through a contractual agreement with certified Adult Day Service Centers in our community and are funded through the Home and Community Care Block Grant, Family Caregiver Support Program and Consumer Directed Services Program.

Adult Day Services are designed to meet the daytime needs of participants who have physical and/or memory related disabilities, or who live at home and depend on support from a caregiver. These services promote independence by allowing the participants to continue to live at home because daytime supervision is available while the primary caregiver is able to work outside of the home. Respite assistance is also provided.

Personal care, meals, health screenings, recreational/leisure activities and rehabilitation services may be provided. Services are available to participants with Dementia and/or Alzheimer’s disease.

CAREGIVER SUPPORT

The Caregiver Support program provides educational and training opportunities and respite for caregivers that will assist them with the necessary knowledge and resources to provide the level of support that their loved one deserves. It also provides access to available services that will help them meet the diverse needs for their specific circumstances. Services may also be available for grandparents raising grandchildren.

The Cabarrus County Caregiver Support program receives federal funding from the Older American’s Act and operates in collaboration with several local agencies that provide support to local caregivers.

CONSUMER-DIRECTED SERVICES

A new program with the Cabarrus County Senior Center is helping some older adults and disabled persons continue to live at home instead of entering a nursing home or assisted living facility. Consumer-Directed Services allows older adults to have direct input and more flexibility for receiving personal care services. Consumer-Directed Services clients can choose for themselves those services which will help them continue to be independent, in addition to personally selecting in-home aides and other service providers.

Each client receives a monthly budget that is used to secure the necessary services and/or supplies necessary to meet the priority needs identified in their personal care plan. They will work closely with their Care Advisor to develop this plan and oversee their own services or supports.

Cabarrus County Senior Center is proud to be one of the only four organizations in North Carolina to receive funding from the North Carolina Department of Health and Human Services to develop this pilot project that provides Consumer-Directed Services. We partner with Cabarrus County Social Services to participate in the Real Choice Systems Change Grant pilot.

FAMILY FRIENDS

Cabarrus Family Friends is an intergenerational volunteer program that matches older adult volunteers with families of children with disabilities and/or chronic illnesses. This program targets volunteers that are 55 years old or older and serves the families of children age 16 and younger.

This program is funded by a grant from the National Council on the Aging and the U. S. Administration on Aging.

Cabarrus Family Friends continuously needs both senior volunteers and families that would like to receive the services of a volunteer to assist them with their individual needs.

HEALTH AND WELLNESS

The Cabarrus County Senior Center offers many opportunities to participate in a diverse selection of wellness programs. Through our Healthy Lives – Healthy Futures program we coordinate a variety of classes, such as Quit Smart smoking cessation classes, fitness classes, nutrition classes, weight management classes, PACE (People with Arthritis Can Exercise) classes and walking clubs. We work in close collaboration with the NorthEast Medical Center, Cabarrus Health Alliance, the Community Free Clinic and the Cabarrus County Parrish Nurse program to provide these programs.

The Cabarrus County Senior Center also coordinates the annual Senior Health and Wellness Day event. Senior Health and Wellness Day is growing into a festival, where older adults in Cabarrus County and beyond gather to learn more about products, services and programs available to them while enjoying fun fellowship among their peers. It is truly one of the most successful Senior Health Fairs anywhere. Our participation passed the 1,000 mark in 2005, and we expect an even more successful event on March 29, 2006 at the Cabarrus Arena and Event Center.

INFORMATION AND ASSISTANCE SERVICES

The Cabarrus County Senior Center provides information on available services and/or resources that may provide assistance or support to local older adults. This service provides information and helps individuals access the services that are needed. Assessments are provided to help older residents of Cabarrus County or their caregivers identify their specific needs and then refer services to help meet those needs. A personal care plan is developed to enroll older adults in the available services.

IN-HOME RESPITE SERVICES

In-home aides are available to provide temporary relief to 24-hour caregivers who are caring for a loved one who cannot be left alone due to physical or mental impairments. To receive this service, either the caregiver or the care receiver must be at least 60 years old.

NUTRITION SERVICES

The Cabarrus County Senior Center offers three different nutrition programs for local older adults.

The LunchPlus Club proudly sports the “More Than Just A Meal” motto as we provide four different programs throughout Cabarrus County. This program is located in Concord, Harrisburg, Kannapolis and Mt. Pleasant. The LunchPlus Club is an overall wellness program that provides a variety of fitness classes, educational presentations and/or classes, leisure/recreational programs, and serves a balanced meal to Cabarrus County residents that are at least 60 years old.

The Supplemental Nutrition program provides a liquid supplement (Boost or Choice) to clients that are unable to eat solid foods. This program requires a referral from the participant’s doctor. You must be at least 60 years old and live in Cabarrus County to be eligible for this service.

The LINCS (Linking Information and Nutrition to Cabarrus Seniors) program provides shelf-stable meals to clients that are unable to attend one of the LunchPlus Club programs and do not receive home-delivered meals from Cabarrus Meals on Wheels.

TITLE V
Older American Community Service Employment Program (OACSEP)

Title V, or the Older American Community Service Employment Program (OACSEP), is a training program for those 55 years of age or older, who live in Cabarrus County and meet certain income requirements. Eligibility is determined at the time of application.

A participant in Title V will receive hourly wages, skill development in new occupations, meaningful work experiences while receiving on-the-job training and personalized assistance in finding employment. Qualities the older worker can bring to the job are a superior work ethic, employer loyalty, maturity in the workplace, a flexible schedule and an eagerness to learn new technologies.

Title V is a program of the U.S. Department of Labor, administered by the State of North Carolina and coordinated locally by the Cabarrus County Department of Aging.

VOLUNTEER SERVICES

Volunteer opportunities are available in all programs to provide support to our staff and the participants that we serve. These volunteers not only assist with Senior Center sponsored programs and events but also with other local agencies and organizations that need help.
